What Happens If You Ignore a Cavity?

Ignoring a cavity allows decay to spread deeper into the tooth, which leads to pain, infection, and eventually tooth loss. A cavity that starts small and painless can turn into a dental emergency within months. The good news is that catching it early almost always means a simpler, faster fix.

How a Cavity Progresses When Left Untreated

A cavity forms when bacteria break down the enamel, the hard outer layer of a tooth. At first, the damage stays shallow and often causes no symptoms at all.

Once decay reaches the dentin, the softer layer beneath the enamel, it spreads faster. This is usually when sensitivity to hot, cold, or sweet foods starts to show up. If the cavity keeps progressing, it eventually reaches the pulp, where the tooth’s nerves and blood vessels live, and that stage brings real pain.

Warning Signs the Decay Is Getting Worse

Cavities rarely jump straight from painless to severe. There are usually warning signs along the way that are easy to miss if you are not looking for them.

  • Sensitivity: a sharp or lingering reaction to hot, cold, or sweet foods often means decay has reached the dentin.
  • Visible changes: a dark spot, rough patch, or small hole on the tooth surface can signal active decay.
  • Pain when chewing: discomfort while biting down often points to a cavity affecting a specific tooth.
  • Bad breath or taste: a lingering odor or off taste can mean bacteria are breaking down tooth structure.

According to the CDC, about 1 in 5 adults between the ages of 20 and 64 have at least one untreated cavity. This is a common problem, not a rare one. Noticing any of these signs is a good reason to schedule a cleaning and exam before the decay advances further.

Risks of Infection and Tooth Loss

Left alone long enough, a cavity can lead to a dental abscess, which is a pocket of infection at the root of the tooth. Abscesses cause swelling, fever, and pain that can spread beyond the mouth if bacteria reach the bloodstream. At that point, a simple filling is no longer an option.

Advanced decay also weakens the tooth’s structure. What could have been treated with a filling may now require a crown or a root canal, and in severe cases, extraction becomes the only choice. Digital X-rays help catch decay before it reaches this point, often before any pain appears.
